The Position: We’re looking for Mental Health Case Managers. The pay range we’re offering is $16.35 - $20.11, depending on education and experience.

Requirements:
Hold a bachelor’s degree in a behavioral science/related field, from an accredited college / university and have completed 2000 hours of supervised experienced in the delivery of services to persons with mental health illness or alternatively receive required DHS approved training upon hire.
Responsibilities:
Complete functional assessments on clients.
Develop the client’s Individual Community Support Plan.
Help the client become integrated in the community through appropriate referral and linking activities.
Monitor and coordinate care with all providers.
Develop a crisis/safety plan for clients.
Stay in compliance with charts and timeliness of note completion.
Represent the agency in a positive light by being respectful toward clients and co-workers.
Perform in an ethical manner and maintain professional boundaries.
The Location: Solutions is based in Moorhead, MN. We also have offices in Fargo, Detroit Lakes, Fergus Falls, Alexandria and St. Cloud. We are currently hiring for full-time Case Managers at our Moorhead office.
